Excuse Notes/Documentation

At Cozy we pride ourselves on providing exceptional Mental Wellness care  to our patients. We understand that sudden changes in our mental health can significantly impact daily responsibilities and may require time for healing, emotional rest and recovery. Upon request we provide a Doctors excuse note from work, school or travel  based on evaluation.
